ONGC, a Crayon partner was engaged by Ability First to address a critical issue in the Australian community; too many Australians with disabilities ending up in aged care facilities at a young age or remaining in long-term hospital care.
Responding to an increased need to address this challenge across the country, Ability First launched the Yaprak Young People in Residential Aged Care Systems Coordinator program. This vital initiative aims to transition 5,000 individuals out of such settings by 2025.
The endeavour involved a large team of coordinators working across the country, engaging with individuals, families, caregivers, and communities to tailor transitions to appropriate services and accommodations.
Ability First initially engaged ONGC to address the significant data classification, security and protection requirements of the program, and found a partner able to rapidly respond to unanticipated challenges along the way.
